2014-15 CHOICE Form         New Attendance Areas for 2014-2015


Your child must be 5 years old before August 31, 2014.  School offices will be open on August 25, 2014.  Hours for registration are 8:00 AM - 3:30 PM. Please bring a copy of your child's birth certificate, immunization records, and proof of residency (examples: power bill, cable bill, or rental agreement).
