SCHOOL BOARD POLICY NO. 4060  DISTRIBUTION OF MATERIALS

Distribution of Materials

The board recognizes that businesses and organizations may want to display or distribute materials in the school district advertising activities or contests that have social, recreational, or educational value for students and parents. All items for display or distribution must have contact information for the organization including phone, mailing address, and responsible person.

The preference is that materials are displayed and made available on public announcement boards or displays. Items for this type of distribution may be approved by the building principal. 

If a group desires a document be sent home with students, the document must be approved by the Superintendent’s Office and must be in English and Spanish. The Superintendent’s Office may limit the number of times an organization can send documents home to twice a year.


Materials shall not:

  • interfere with or inhibit classroom instruction or any school function
  • promote the use of illicit drugs, alcohol, or tobacco
  • promote hostility, disorder, or violence
  • discriminate as described in Federal Title IX Guidelines
  • be libelous
  • be of a sexual nature
  • proselytize or disparage religious beliefs
  • promote or oppose any political candidate or cause

Standard Tort Claim Form 


Washington state law (Chapter 4.92 RCW) requires a Standard Tort Claim Form to be submitted when filing a tort claim against the Eastmont School District.  Standard Tort Claim forms cannot be submitted electronically (via email or by fax), but must be mailed or delivered to the address noted below.


Required By Law
Engrossed Substitute House Bill 1553, effective July 26, 2009, requires citizens to present the Standard Tort Claim form with the government agency named in their claim. The law also requires State and local government agencies to post the Standard Tort Claim form on their websites with instructions on how to complete the form. In compliance with these requirements and for the convenience of citizens.


Legal Requirements for Presenting Standard Tort Claim Forms
In order to verify the claim and additional supporting information, the law requires that the Standard Tort Claim form be signed by:

•     Claimant; or

•     Person holding a written power of attorney from the Claimant; or

•     Attorney in fact for the Claimant; or

•     Attorney admitted to practice in Washington State on the Claimant’s behalf; or

•     A court‐approved guardian or guardian ad litem on behalf of the Claimant

Instructions for Completing a Tort Claim Form

  • Before presenting a Standard Tort Claim form, please read these instructions, the Standard Tort Claim form, and other appropriate forms in their entirety.
  • Type or print clearly in ink and sign the Standard Tort Claim form.
  • Provide all requested information and any available documents or evidence supporting your claim, such as medical records or bills for personal injuries, photographs, proof of ownership for property damages, receipts for property value, etc.
  • If the requested information cannot be supplied in the space provided, please use additional blank sheets so your Standard Tort Claim form can be easily read and understood.
  • The following are examples on how to complete the Standard Tort Claim Form (#SF 210):

              14.      List all other witnesses having knowledge of the incident in question, with their names, addresses, and telephone numbers that are not listed within items 11 and 12. Also include a description of their knowledge. For example, if your sister was with you, when the alleged incident occurred, please include her name, address, telephone number, and indicate she witnessed the incident.

              15.      Please describe the incident that resulted in the injury or damages, specifically answering the questions who, what, where, when and why.

              16.      If you reported this incident to law enforcement, safety, or security personnel, please provide a copy of the report or contact information to the person you spoke with.

              17.      Please provide all of your medical providers with their names, address, telephone numbers, and the type of treatment. If you were treated for a personal injury, please include your medical records and bills.

              18.      Attach documents.

              19.      Please provide the dollar amount for your damages, including your time loss, medical costs, property damage loss, etc. This amount should represent your opinion of total compensation.

Helpful Information At a Glance

Requests to use Eastmont School District’s facilities and fields are now processed on a seasonal basis.  Applications will be accepted on the opening date or on the first business day following the opening date (Monday-Friday).  Application periods are defined as follows:

Application Process for Using Athletic Facilities (Gyms and Fields)

  1. A complete Facility Use Application, accompanied by a valid Certificate of Insurance and if applicable, an IRS Determination letter regarding nonprofit status, must be submitted to the District Facility Use Manager Russ Waterman at least 10 business days prior to the scheduled event.  This requirement applies to ALL classifications of users. 

  2. Eastmont School District requires a Certificate of Liability Insurance with the following requirements:

                   a.  $1,000,000 of General Liability

                   b.  Eastmont School District listed as Certificate Holder

                   c.  Eastmont School District listed as an Additional Insured


Application Process for Using a Building Facility (Auditorium, Library, or Classroom)

  1. A complete Facility Use Application, accompanied by a valid Certificate of Insurance and if applicable, an IRS Determination letter regarding nonprofit status, must be submitted at least 10 business days prior to the scheduled event.  This requirement applies to ALL classifications of users.

Scheduling Priority

The priority levels for scheduling use of school facilities or field use are as follows:

 

  1st Priority:

  Eastmont School District (Group 0):

  a.  Eastmont School District (K-12) classes and programs.

  b.  Eastmont School District sponsored extra-curricular events
        (open houses, athletic practices/events, arts and cultural                 
        performances, parent meetings, etc.).

  c.  District sponsored community classes, programs, and activities

nd Priority:

  Government and Nonprofit Users that provide services primarily to   
  youth that reside within the boundaries of Eastmont School District
  (Group 1).


  3
rd Priority:

  Government and Nonprofit Users that provide services primarily to
  youth that do not reside within the boundaries of Eastmont School 
  District (Group 2).

  4th Priority:

  Government and Nonprofit Users that provide services primarily to
  adults (Group 3).

  5th Priority:

  Commercial/Profit making organizations and Private Events
  (Group 4).
